In my dark past, I wrote two 'shareware' utilities that allow you to convert between MIDI and ProTracker (MOD) music formats. They are released with accompanying source code, since I'm lazy enough not to want to maintain my code forever.

MidiMod (version 0.2) converts MOD files to MIDI ones. (documentation)
PtMid (version 0.3) converts MIDI back to MOD. (documentation)
